All admissions to Hospice Ministries must meet the established admission criteria and be approved by the patient's attending physician and the Hospice Medical Director.
To be eligible for Hospice Services the potential patient and/or family (where appropriate) must:
Have been diagnosed with a terminal illness and have been given a life expectancy of six months or less.
Have a primary physician who agrees to Hospice care for the patient.
Be assessed for needs and desire for Hospice services prior to initiation of services.
Be assessed by the Hospice Team as able to utilize the services prior to initiation of services.
Have a primary caregiver in the home when needed who is able and will be responsible for following directions of the Hospice Team including proper administration of medication.
Sign an informed consent (by patient or caregiver ) to receive palliative (relief of pain/reduction of symptoms), rather than curative, forms of care.
